Health and safety / medical necessity

Before applying for lifestyle coaching and training with Elizabeth Mucklow, the client must confirm that he or she does not have or suffer from any condition, illness, disability, weakness or any other illness that could affect or prevent his/her participation in any form of exercise, with or without equipment, or his/hers nutrient metabolism. The client is hereby advised to consult with a registered health professional to ensure that he/ she can physically tolerate Elizabeth Mucklow's services before commencing its use. Elizabeth Mucklow will use her best efforts to ensure that all her services are provided in accordance with reasonable health and safety standards through precautionary measures. If you decide to participate in a service related to Elizabeth Mucklow without your physician's approval, you hereby assume all responsibility for your participation. You hereby acknowledge that you understand that any fitness-related advice provided by Elizabeth Mucklow is not for a medical necessity or a mandatory prescription, and is therefore not intended for the management or treatment of any health condition or condition, but for educational purposes and therefore does not include any medical advice. By applying for Elizabeth’s coaching you waive the right to take legal action against Elizabeth Mucklow.

 

Risk of exercise/diet changes

A person must hereby recognize that exercise (including, but not limited to strength, aerobic, and flexibility exercises with and without the use of fitness equipment) and diet changes can be associated with adverse health effects and risks, such as injury and even death. With knowledge of the hazards involved, clients voluntarily participate in these activities and agree to accept all risks of injury or death related to the fitness activities listed. By signing up for fitness/lifestyle coaching/dance the client hereby waives, releases, and discharges it forever and hereby releases Elizabeth Mucklow from all costs and expenses, responsibility, liability including damage or injury, whether foreseeable or not, associated with the client's participation in services related to, but not limited to, nutrition, his/her personal health, his/her use of information or materials designed or provided by Elizabeth Mucklow.   Hereby also Elizabeth Mucklow is released from any responsibility or liability for any adverse health effects, injuries, damages or conditions to his/her self, whether foreseeable or not, of any activities related to Elizabeth Mucklow and any information provided by her.

 

Responsibility for results of all coaching programs of Elizabeth Mucklow

When participating in any coaching program of Elizabeth Mucklow, the client must agree that his or her results are their sole responsibility and are not guaranteed by purchasing and/or participating in any products, programs or training. Therefore, the client waives Elizabeth Mucklow's liability for any or all of his/hers results from all coaching programs or lack thereof.

 

Payments

The client agrees to prepay in full for all services provided by Elizabeth Mucklow, prior to the estimated start date stated on the invoice. For payments in instalments, the client agrees to pay each instalment by the due date. If payments are not received in full, Elizabeth Mucklow is under no obligation to provide the client with any service.

 

Cancellation Policy

 If a client does not cancel a booked and confirmed training or coaching session at least 24 hours before the start time with written proof or does not cancel/am not present at all, the full amount will be charged for that session. In addition, Elizabeth Mucklow may cancel any scheduled service that has not been prepaid in full by the date specified on the invoice until payment is made.

Your personal information is controlled and managed by Elizabeth Mucklow. Your personal data may be used for the sole purpose of providing you with services purchased by or for you from Elizabeth Mucklow. Your personal data may also be used for the sole purpose of providing you with information about Elizabeth Mucklow's services.
